Please Use This Form for a request of Cemetery Records.  Information available through this search may include:  Date of death, relatives of the deceased and location of the burial site.
 
For Death Certificates please contact the Henry County Health Department at (765) 521-7058.  The City of New Castle does not have death certificate information.
